Matched pair analysis of the effect of longer hypothermic machine perfusion time on kidney transplant outcomes

This single-center study published in World Journal of Transplantation looked at deceased donor kidney transplant pairs occurring at University of Saskatchewan between 2012 and 2021. Both donor kidneys were placed on LifePort Kidney Transporter prior to transplant. Kidneys transplanted first (Kidney 1) had a median pump time of 5.2 hours, and those transplanted second (Kidney 2) had a median pump time of 10.8 hours.

The investigators found:

  • Higher incidence of DGF in Kidney 1 compared to Kidney 2
  • Freedom from biopsy-proven acute rejection was observed at a higher incidence in Kidney 2
  • Graft survival was longer for Kidney 2 compared to Kidney 1.

The authors hypothesize that longer pump times offer additional anti-inflammatory factors and reduce vasoconstricting peptides, allowing cells and tubules to better recover from ischemic injury.
