Use of Machine Perfusion to Increase the Number of Expanded Criteria Deceased Donor Kidney Transplants: A Pharmacoeconomic Analysis

A recent pharmacoeconomic study from Brazil by Tedesco Silva Junior et al. sought to evaluate the cost-effectiveness and budgetary impact of incorporating hypothermic machine perfusion (HMP) for preserving extended criteria donor (ECD) kidneys.   

They found that using HMP for ECD kidneys and static cold storage (SCS) for standard criteria donors:  

  • Increased transplants by 1123 
  • Decreased patients on dialysis by 815 
  • Decreased patient deaths by 120 
  • Incurred an additional cost of 4,453,749 USD, but on budget impact analysis realized resource savings by year 5

Overall, they concluded that using HMP for ECD kidneys is a cost-effective strategy. They found that they could increase the number of transplants, reduce the number of patients on dialysis, and decrease the mortality rate with a reasonable cost-effectiveness ratio.
